Referral for Assistive Technology Assessment

If during the consideration process the IEP team determines that additional information is needed to make informed decisions regarding assistive technology devices or services then a referral for an AT assessment would be necessary. The referral form should contain background information and outline specific areas in which the student is having difficulty and strategies that are currently in place.

The following referral form is provided as a sample for school divisions to use when developing procedures for AT assessment.
